The Hospice Physician will provide physician services by acting as a medical resource to the interdisciplinary group, assuring appropriate measures to control patient symptoms, and complementing attending physician care. The Hospice Physician will serve as a hospice champion-promoting and representing the program to physicians, physician groups, discharge planners, other referral sources, community health organizations and potential donors as appropriate.

  • Devotes to the best of their ability to the proper management of the program assuring that the established policies, bylaws, rules and regulations of the organization are followed.
  • Assures established policies, bylaws, rules, and regulations of the organization are followed in the program.
  • Adheres to requirements, terms, and conditions required by Medicare Conditions of Participation, accreditation organization, and federal and state statutes governing the provision of services.
  • Provides training regarding the medical aspects of caring for terminally ill patients to physicians, personnel, and volunteers.
  • Participates in the interdisciplinary plan of care reviews, patient case review conferences, comprehensive patient assessment and reassessment, and the quality improvement and utilization.
  • Reviews patient’s medical eligibility for hospice services, in accordance with hospice program policies and procedures, and establishing the plan of care in conjunction with attending physician and interdisciplinary group prior to providing care including written certification of terminal illness.
  • Acts as primary physician for patients who referring/attending physicians desire to relinquish that care and/or if the referring/attending physicians are not available for further contact.


  • Maintain licensure, in good standing, as a licensed physician in the state preforming over-site in.
  • Three (3) to five (5) years' experience in a health care setting.
  • One (1) year experience in hospice care team or in another setting managing the care of terminally ill patients.. Preferred but not mandatory.
  • Maintains DEA registration and all controlled substance authorizations with state and federal authorities.
  • Has experience in Hospice or palliative care and/or training in end-of-life care. Board certified in Hospice and Palliative Care preferred.
