Hospice of the West is currently seeking a  Full Time Hospice Intake Coordinator to join our team in Chandler.  If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!   HOURS: Tuesday - Friday 9A - 6P & every Saturday 8A - 5P   The Hospice Intake Coordinator is responsible for all aspects of the patient intake and admission processes including directing and coordinating and initiation of hospice services, and any other equipment, supplies, external services for hospice clients as prescribed by the physician, and/or requested by the client/family. Hospice Intake Coordinators respond to all incoming and outgoing physician and payer communication (insurance verification and authorization), and are in frequent communication with hospice clinical staff.
